Glucose Intake and Utilization in Pre Diabetes and Diabetes Implications for Cardiovascular Disease

by Ronald Ross Watson (Editor), Betsy Dokken (Editor)This important reference, edited by Ronald Ross Watson and Betsy Dokken, collects the research needed to make the distinct connection between pre-diabetes, diabetes, and cardiovascular disease. Glucose Intake and Utilization in Pre-Diabetes and Diabetes: Implications for Cardiovascular Disease explains the mechanisms of progression from pre-diabetes to diabetes to cardiovascular disease. Since pre-diabetes and diabetes are important cardiovascular disease risk factors, and impaired glucose metabolism among cardiac patients is extremely prevalent, the importance of reviewing pre-diabetes and its involvement in CVD complications is vital as one applies food and glycemic control to slow progress to diabetes and heart disease. The book further focuses on glucose intake and utilization in diabetes, including coverage of diabetes in the development and pathology of cardiovascular disease, risks and epidemiology of cardiovascular problems promoted by diabetes, macrovascular effects and their safety in therapy of diabetics, beta cell biology and therapy of diabetes, and nutrition to modulate diabetes.
    • Offers a complete review of cardiac health problems occurring with significant frequency in patients relative to their ability to regulate glucose
    • Presents coverage of the role of glucose utilization, development of pre-diabetes and the ultimate development of various cardiovascular diseases
    • Provides thorough dietary, nutrition, complementary and alternative botanical therapies for pre-diabetes and diabetes to halt the progression to cardiovascular disease

Adverse Cutaneous Drug Reactions to Cardiovascular Drugs

by Esen Özkaya (Author), Kurtuluş Didem Yazganoğlu (Author)
Adverse cutaneous drug reactions (ACDR) are among the most frequent events in patients receiving drug therapy. Cardiovascular (CV) drugs are an important group of drugs with potential risk of developing ACDR especially in elderly as marketing of more new drugs and their prescription continue to increase. However, like with most other drugs the exact incidence of cutaneous side effects from CV drugs is difficult to estimate due to sporadic reporting. Moreover, a reliable designation of a certain drug as the cause of a certain type of reaction can rarely be made. Apart from the well-known angioedema/urticaria from ACE inhibitors, lichen planus / lichenoid reaction from beta adrenergic blockers and photosensitivity from thiazide diuretics, ACDR from CV drugs might be seen in a wide spectrum extending to rare but life-threatening conditions such as erythroderma, Stevens-Johnson syndrome, toxic epidermal necrolysis or drug hypersensitivity syndrome. In this comprehensive review, the reported types of ACDR to CV drugs will be discussed according to drug class and the type of dermatologic reaction with special emphasize on cross-reactions and the role of patch testing in diagnosis.

The Cardiovascular System Physiology Diagnostics and Clinical Implications

The cardiovascular system includes the heart located centrally in the thorax and the vessels of the body which carry blood. The cardiovascular (or circulatory) system supplies oxygen from inspired air, via the lungs to the tissues around the body. It is also responsible for the removal of the waste product, carbon dioxide via air expired from the lungs. The cardiovascular system also transports nutrients such as electrolytes, amino acids, enzymes, hormones which are integral to cellular respiration, metabolism and immunity. This book is not meant to be an all encompassing text on cardiovascular physiology and pathology rather a selection of chapters from experts in the field who describe recent advances in basic and clinical sciences. As such, the text is divided into three main sections: Cardiovascular Physiology, Cardiovascular Diagnostics and lastly, Clinical Impact of Cardiovascular Physiology and Pathophysiology.

Vortex Formation in the Cardiovascular System

by Arash Kheradvar  (Author), Gianni Pedrizzetti  (Author)Vortex Formation in the Cardiovascular System will recapitulate the current knowledge about the vortex formation in the cardiovascular system, from mechanics to cardiology. This can facilitate the interaction between basic scientists and clinicians on the topic of the circulatory system. The book begins with a synopsis of the fundamentals aspects of fluid mechanics to give the reader the essential background to address the proceeding chapters. Then the fundamental elements of vortex dynamics will be discussed, explaining the conditions for their formation and the rules governing their dynamics. The main equations are accompanied by mathematical models. Cardiovascular vortex formation is first analyzed in physiological, healthy conditions in the heart chambers and in the large arterial vessels. The analysis is initially presented with an intuitive appeal grounded on the physical phenomena and a focus on its clinical significance. In the proceeding chapters, the knowledge gained from either clinical or basic science literature will be discussed. The corresponding mathematical elements will finally be presented to ensure the adequate diligence. The proceeding chapters ensue to the analysis of pathological conditions, when the reader may have developed the ability to recognize normal from abnormal vortex formation phenomenon. Pathological vortex formation represents vortices that develop at sites where normally laminar flow should exist, e.g. stenosis and aneurisms. This analysis naturally leads to the interaction of vortices due to the surgical procedures with respect to prediction of changes in vortex formation. The existing techniques, from medical imaging to numerical simulations, to explore vortex flows in the cardiovascular systems will also be described. The presentations are accompanied by the mathematical definitions can that be understandable for reader without the advanced mathematical background, while an interested reader with more advanced knowledge in mathematics can be referred to references for further quantitative analyses. The book pursues the objective to transfer the fundamental vortex formation phenomena with application to the cardiovascular system to the reader. This book will be a valuable support for physicians in the evaluation of vortex influence on diagnosis and therapeutic choices. At the same time, the book will provide the rigorous information for research scientists, either from medicine and mechanics, working on the cardiovascular circulation incurring with the physics of vortex dynamics.

Principles and Practice of Geriatric Medicine 5th Ed

by David J. Stott (Editor), Gordon D. O. Lowe (Editor)As improvements in living conditions and advances in modern medicine lead to increased life expectancy and the number of older adults world-wide continues to grow, so the number of older individuals with cardiac and cerebrovascular disease is rising. Over eighty per cent of all cardiovascular disease-related deaths now occur in patients aged sixty five or older.  The high prevalence of co-morbidities, frailty and cognitive decline amongst older patients requires that a holistic approach to the management of cardiovascular disease is required.Written and edited by leading experts in the field, ‘Cardiovascular diseases and health in the older patient’ provides scholarly evidence and data which explain why treating an older patient with cardiovascular disease is often different from treating a young or middle-aged patient with the same disease.  The chapters have been updated and expanded from the cardiovascular section of the renowned ‘Pathy’s Principles and Practice of Geriatric Medicine’, edited by Alan J. Sinclair, John E. Morley and Bruno Vellas.  The revised chapters contain updated evidence and references to recent clinical practice guidelines in Europe and North America, this book provides complete, authoritative information on this major cause of death, poor health and disability in old age.Covering the epidemiology, pathophysiology and management of cardiovascular disease in the older patient, the book is an excellent reference at all clinical and pre-clinical levels and will appeal particularly to geriatricians, cardiologists, and GPs, as well as cardiac specialist nurses and practice nurses. With a Foreword by Stuart M. Bioactive Food as Dietary Interventions for Cardiovascular Disease Bioactive Foods in Chronic Disease States

by Ronald Ross Watson (Editor), Victor R Preedy (Editor) One major example of the synergy of bioactive foods and extracts is their role as an antioxidant and the related remediation of cardiovascular disease. There is compelling evidence to suggest that oxidative stress is implicated in the physiology of several major cardiovascular diseases including heart failure and increased free radical formation and reduced antioxidant defences. Studies indicate bioactive foods reduce the incidence of these conditions, suggestive of a potential cardioprotective role of antioxidant nutrients.Bioactive Food as Dietary Interventions for Cardiovascular Disease investigates the role of foods, herbs and novel extracts in moderating the pathology leading to cardiovascular disease. It reviews existing literature, and presents new hypotheses and conclusions on the effects of different bioactive components of the diet.
    • Addresses the most positive results from dietary interventions using bioactive foods to impact cardiovascular disease
    • Documents foods that can affect metabolic syndrome and other related conditions

A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases

by Committee on a National Surveillance System for Cardiovascular and Select Chronic Diseases (Author), Institute of Medicine (Author)Chronic diseases are common and costly, yet they are also among the most preventable health problems. Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data--including population surveys, cohort studies, disease registries, administrative health data, and vital statistics--contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts.A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.

by Shankar Sridharan (Author), Gemma Price (Author), Oliver Tann (Author), Marina Hughes (Author), Vivek Muthurangu (Author), Andrew M. Taylor (Author)
The last 10 years has seen explosive expansion of the number of centres performing cardiov- cular magnetic resonance (CMR) imaging. The majority of this expansion has been in the ? eld of adult ischaemic imaging, but congenital heart disease remains one of the main indications for CMR. Importantly, the greatly improved survival of patients with congenital heart disease gives us a burgeoning adult population living with the sequelae of the disease (grown-up c- genital heart disease – GUCH). Without previous experience or formal training, the interpretation of CMR images of patients with congenital heart disease can be dif? cult. The main aim of this book is to create a portable resource that offers ef? cient access to high-quality MR (and where appropriate, CT) images of the common congenital and structural heart abnormalities. We hope that by prov- ing key images for each condition and a clear interpretation of the MR appearances, we will improve the reader’s understanding of the conditions, facilitate their interpretation of images and optimise the planning of the imaging protocols during their own practice of congenital CMR. As with any publication from a single institution, the contents of this book represent our own practice. We have not written a de? nitive or exhaustive description of the conditions.

    Cardiotoxicity of Non Cardiovascular Drugs

Some drugs which are not aimed at treating heart disease have nevertheless been found to have profound effects on heart muscle. Cardiotoxicity is one of the major forms of toxicity seen in drugs and it accounts for most drug recalls and delays experienced in regulatory approvals. In recent years a number of non-cardiac blockbuster drugs such as terfenadine have been withdrawn from major markets because of cardiotoxicity concerns, while other drugs have either been withdrawn prior to marketing or required labelling changes that significantly restricted their use. In Cardiotoxicity of Non-Cardiovascular Drugs international experts describe the molecular mechanisms and clinical read-outs of cardiac events induced by a broad variety of noncardiovascular drugs. Particular emphasis is paid to the preclinical screening of drug cardiotoxicity. Topics include:
    •  metabolic targets of cardiotoxicity
    • regulatory aspects
    • translating molecular mechanisms into clinical trials
    • structure-activity relationships in arrhythmias by antihistamines and psychoactive drugs
    • cardiovascular toxicity of antitumor drugs
    • cardiovascular toxicities of non-steroidal anti-inflammatory drugs
    • cardiovascular toxicities of antiretroviral therapies
Cardiotoxicity of Non-Cardiovascular Drugs is an essential guide to this important area of drug development. It will find a place on the bookshelves of researchers, regulators and students in medicinal chemistry, drug development, pharmacology, pharmacy and cardiovascular disease

Studies on Studies on Cardiovascular Disorders

by Heinrich Sauer (Editor), Ajay M. Shah (Editor), Francisco R. M. Laurindo (Editor)The role of reactive oxygen species (ROS) in the cardiovascular system is Jan- faced. Whereas low concentrations of ROS are involved in variety of physiological signalling events, oxidative stress resulting from deregulated overproduction of ROS and/or impaired antioxidant defences contributes to cardiovascular disease. The actions of ROS in the cardiovascular system are a fascinating topic, not only for the basic science researcher but also for the clinician who is interested in seeking new therapies for his patients suffering from cardiovascular disease. The current book provides a comprehensive overview of the molecular mechanisms and pathoph- iological settings in which chronic and detrimental oxidative stress arises within the heart and vasculature. The book also considers currently discussed strategies in avoiding chronic redox stress resulting from exposure to risk factors or various cardiovascular interventions. The series starts with an overview by Denise de Castro Fernandes, Diego Bonatto and Francisco Laurindo of redox signaling models that could underlie the dev- opment of redox-associated cardiovascular disorders. The interactions of proteins within signalling cascades with ROS and the regulation of such interactions by the anti-oxidative capacity of the cell are discussed. Rebecca Charles, Joseph Burgoyne and Philip Eaton report on redox-mediated modi cations of proteins under ph- iological and pathophysiological conditions and the variety of post-translational oxidative modi cations that explain redox sensing and signal transduction by proteins at the molecular level. ROS are generated during embryogenesis and may be involved in the proper development of the cardiovascular system.

Renin Angiotensin System and Cardiovascular Disease

by Walmor C. DeMello (Editor), Edward D. Frohlich (Editor)Experimental and clinical evidence supports the view that the activation of the renin angiotensin aldosterone system is involved in cardiovascular pathology including hypertension, heart failure, myocardial ischemia, and atherosclerosis. The present volume describes the intricacies involved in these processes, including the in?uence of prorenin/renin, angiotensin II, angiotensin (1-7), and aldosterone on cardiac and vascular functions as well as their involvement in the generation of cardiovascular diseases. Fundamental aspects like intracellular signaling, regulation of cell volume in the failing heart, and the presence of an intracrine renin angiotensin system are discussed. Moreover, the role of the mineralocorticoid receptor as an important c- ponent of the intracrine renin angiotensin system and as a regulator of extracellular action of angiotensin II is described, reinforcing the view that aldosterone inhibitors are helpful in the treatment of heart failure and hypertension. Let us hope the imp- tant topics included here motivate basic and clinical investigators and contribute to the development of new therapeutic approaches for cardiovascular diseases. We want to thank the distinguished authors and Humana Press for the opportunity to publish this important book. Walmor C. Patient Specific Modeling of the Cardiovascular System Technology Driven Personalized Medicine

by Roy C.P. Kerckhoffs (Editor) Peter Hunter Computational physiology for the cardiovascular system is entering a new and exciting phase of clinical application. Biophysically based models of the human heart and circulation, based on patient-specific anatomy but also informed by po- lation atlases and incorporating a great deal of mechanistic understanding at the cell, tissue, and organ levels, offer the prospect of evidence-based diagnosis and treatment of cardiovascular disease. The clinical value of patient-specific modeling is well illustrated in application areas where model-based interpretation of clinical images allows a more precise analysis of disease processes than can otherwise be achieved. For example, Chap. 6 in this volume, by Speelman et al. , deals with the very difficult problem of trying to predict whether and when an abdominal aortic aneurysm might burst. This requires automated segmentation of the vascular geometry from magnetic re- nance images and finite element analysis of wall stress using large deformation elasticity theory applied to the geometric model created from the segmentation. The time-varying normal and shear stress acting on the arterial wall is estimated from the arterial pressure and flow distributions. Thrombus formation is identified as a potentially important contributor to changed material properties of the arterial wall. Understanding how the wall adapts and remodels its material properties in the face of changes in both the stress loading and blood constituents associated with infl- matory processes (IL6, CRP, MMPs, etc.
